Immunization Requirements

 

• Tuberculosis (TB test is required within 12 months before first entry into school in Hawaii)

• Varicella vaccine is required for entry into preschool, kindergarten, and 7th grade and for all students entering the Hawaii school ststem for the first time. A documented history of varicella, signed by the Provider, may be substituted for th varicella vaccine requirement.

• Documentation of receipt of the following vacccines is required for 7th grade attendance:
2MMR vaccines
3 hepatitis B vaccines
1or 2 varicella vaccine(s)

• Documentation requirements:
Physical examination must be signed by a Medical Doctor (MD), Doctor fo Osteopathy (DO), Advanced practice Registered Nurse (APRN), or Physician's Assistant (PA), TB examination must have Provider's signature or stamp
Immunization record must be reviewed ans signed or stamped by Provider

• Laboratory evidence of immunity in lieu of a record of immunization must include both a copy of the laboratory report, and a signed statement by the Provider certifying that the student is immunde to the specified disease.

• Medical exemptions to immunizations must state the length of time during which the vaccine would endanger the student's health or life, and must be signed by a US licensed M.D. or D.O.
